Well usually gauze is removed with sterilized instrument like forcep so as to prevent infection .Usually doctors usually keep one end of the gauze near the opening of the wound so that gauze is not lost inside the cut .Also the lesion is cleaned with betadine .At the most the gauze can be removed gently and patient given pain medicine before dressing .I hope this helps
