What are the max dose of lidocaine at 2%, 1%, and .5% w/o epi and the max dose of 1% with Epi? Also why does it burn when you inject it and what can be added to prevent the burn?

Hi,
For normal healthy adults, the individual maximum recommended dose of lidocaine HCl with epinephrine should not exceed 7 mg/kg (3.5 mg/lb) of body weight, and in general it is recommended that the maximum total dose not exceed 500 mg.
When used without epinephrine the maximum individual dose should not exceed 4.5 mg/kg (2 mg/lb) of body weight, and in general it is recommended that the maximum total dose does not exceed 300 mg.
Once given the maximum dose of lidocaine, it should not be repeated for 2 hours.
For the pain, using the injection at room temperature should help.
Also since the pain is due to acidic nature of the solution, adding 1 ml of 1 % sodium bicarbonate may help, however this should be done with the consent and supervision of the attneding doctor,

Where does the 2% and 1% come into play?

Lidocaine 2% contains 20 mg in each ml, while 1% contains 10 mg in each ml.
