How do I find out my passport number and issue date

Resolved Question:

I lost my passport, and need to submit a lost/stolen passport report and apply for a new passport. Both forms require the number and issue date of the old passport. How do I obtain this information?

Unless you have that number written down on a different form, you are going to have to get it from the issuing agency.

When you go to submit your form, you can provide DL and probably a certified birth certificate and they can pull the number and issue date up from thier records.
