On the majority of these trucks we are seeing with an engine noise and rough idle most shops are recommending an engine replacement because they are scared to replace timing sets on them. The problem that you are having is a worn timing chain, cam pahsers, VVT solenoids, and chain guides. We have replaced a number of them on our fleet trucks recently.
The problem is that occasionally the engine is so worn that a new timing set will just accentuate other problems. So the customer ends up paying about $3000 and ends up with more problems. That's why most shops are saying to replace the engine.
