Yes, most of these focuses only have front ABS in 2001. There was a rare option for 4 wheel abs, but it's not common.
In a lot of cases the hydraulic control unit needs to be replaced. They fail fairly often. It could also be a bad wheel speed sensor.
Yes, and if yours has 4 lines coming from it then you do have the 4 wheel anti-lock option. It is shown in this diagram on page 5.
