Your symptoms fit that of a slipping belt, pulley that has a bad bearing, or the tensioner not holding enough tension on the belt. You have addressed some of the pulleys and the tensioner. Try removing the belt and cleaning it with brake cleaner as well as all the pulleys. Use a stick stethoscope and listen to the pulleys or components while running to see if any sound rough.
