I have a 98 Windstar with 148000 on it and when I drive down the street the trans mission jerks from OD to neutral and jumps back an forth causing jerking and as you preess on the accellerator it it acts like its in neutral could this be a problem with the torque converter?

It does sound like the torque converter may be engaging and disengaging making the jerking sensation you are feeling.

If it is the torque converter does that need to be replaced or is it something that can be fixed withiut replacement

The torque converter needs to be replaced as a unit. You may want to look into fist having the computer checked for Diagnostic Trouble Codes or DTC's. This could better help pinpoint the problem.
