I have spoken with the IRS regarding making payments on an amount that I owe. The Representative that I spoke with recently told me that I have to filled out an 433-D form. I was told by the Rep that they were going to sent to me such form but I never received it. Now, I have the form, filled out but I have no address to sent it to. I need an address to sent the Installment Agreement; I reside in High Point, North Carolina. Please help.

Even though you spoke to someone over the phone, If you do owe over $25,000, you should send both the 9465 and 433-F IRS Forms.

...

9465 is the installlment agreement request, and the 433 is, as you know, to collect your data/financial information
