I am trying to change a name on my birth Certificate. My

Customer Question

I am trying to change a name on my birth Certificate. My father is listed as unknown. I found out who is after 57 years. Unfortunately he died in 2015. I wanted to know what procedures and How I get a court order to have the name put on my birth certificate.
JA: Because laws vary from place to place, can you tell me what state the court order was issued in?
Customer: It hasn't been issued but I was born in New York.
JA: Have you talked to a lawyer yet?
Customer: No
JA: Anything else you want the lawyer to know before I connect you?
Customer: How much does it cost?
JA: I'm not sure of the exact price, but there's only a $5 deposit. The rest of the price information will be on the page I send you to.
Customer: Can I get back to you on this?
