HelloThis is Samuel. Do you have a mutual agreement with the other parent in regard to custody and visitation or is there an order from another case which deals with the issue? In other words, what is the reason the court has no jurisdiction at this time?
Expert:  Samuel II replied 6 months ago.
if there is an agreement or other stipulation between the parents, you will mark box 1 under question 4j
Expert:  Samuel II replied 6 months ago.
If no agreement has been signed yet, then you can opt for box 2 and fill out and attach FL 341
Expert:  Samuel II replied 6 months ago.
That will not correct. So let me restate - You will mark box 1 and 2 and attach form FL 341
Expert:  Samuel II replied 6 months ago.
If you have no stipulation, then you choose box 3 and attach form FL 355

If the matter is not an issue because there is a previous court order, then you mark box 4 and attach a certified copy of that court Order.
