I WILL SIGN HIM UP FOR SCHOOL, DRIVERS' LICENSE, AND ALL EVERYDAY THINGS. SHE WILL SUPPORT HIM AND PAY FOR HIS HEALTH INSURANCE. IS A 'CHILD GUARDIAN CONSENT FORM' ADEQUATE?
