The document that states "Order" is the document you follow, since that is the official order and anything that conflicts with that the "order" supersedes. If there are conflicts they also need to be clarified since the Order should not conflict with the other documents in the case and you would do that with a letter to the mediator first and then if he will not clarify you would have to file a motion to clarify order with the court to get a court order to clarify which of the two is correct.
