I am the executor of an irrevocable trust in San Francisco, Ca., I am also a beneficiary. I am in the preliminary process of transferring real property solely to my name. My question is whether to fill out the (affidavit death of trustee) or (affidavit
change of trustee) and what are the significant differences?

the death of trustee form applies. you use the other when trustee resigns or is substituted otherwise. Please let me know if the answer is fast and acceptable.

so once i change the deed into my name, is the property still in the trust?

It would not be. Make sure the trust permits the transfer out of trust, and why you would want it out of trust.
