i have a high TSH level of 7.4 and the new guidlines suggest 0-3.0 levels. With
the suggested hypothyroid diagnosis what dosing should i start on w/ synthroid?
i'm waiting on my T4, free T4 and T3 levels. would you get any other thyroid levels?

yes.

fatigue in afternoon. high lipid panel. increased hair loss. HTN.

Customer :

sorry... my weight is 155. i run marathons and half marathons every weekend

You can wait for the other labs to come. If T4 and T3 are normal, I would suggest repeating labs in a few months, and include thyroid antibodies. If thyroid levels are normal, you have compensated hypothyroidism (higher TSH pushes thyroid gland to make more, you're OK for now). If antibodies are positive, you're probably on the way to developing hypothyroidism, and you should start replacement. Your dose should probably be 50-75 mcg/day, but I would check with your endocrinologist.
