Thank you for the information and your question. It is unusual for the employer/carrier to file this form so, no, it is not just a normal way of doing business. They had to have written the reason for the controversion and the form gives your wife her rights to appeal. She should appeal if she believes the reasons for the controversion are unfounded or incorrect. She may want to speak to a WC attorney about her claim and get help in her process of appeal.
