Hi, in continuation to my above situation, if my friend wants to file an appeal, how is the format to be followed. is it just a formal letter, explaining the issues. thanks

Your friend needs to look at the denial letter she received and there is a number on there and it also tells her how many days she had to appeal from the denial and if she has missed that time, then she would not be able to appeal. She would send a letter explaining the situation to that address or fax number and explain the situation and attach a letter from the doctor that she is fit to work. However, even if she has missed that deadline it is not a big issue in this case, since they claim she was not healthy enough to work at that time and she is now, she can file a brand new claim stating that she is now healthy and willing and looking for work and as such she is now qualified and proceed as a new claim and not appeal the old claim if it was denied based only on her health.
