Is there a notice of separation form that must completed when terminating a Florida employee? If so, how do I get the form?

Florida does not require any particular notice of separation/Termination form to the employee a simple letter is fine or even verbal notice. If you want to use one, here is one from the Florida Government which you can modify, but it is not required under the law. http://www.dcf.state.fl.us/publications/policies/060-70ch1.pdf
