If I were in this situation I would get the healthcare company on the phone.
They're going to make you convince them that you are who you say you are.
Explain the situation and tell them that the email address is so rarely used that
you forgot what it is. Either ask them to give you the old email address or better
yet, see if you can get them to change it on their end to your gmail address.
The first step in this situation is knowing what the email address is and then you
can start fixing this. Without an email address I can't even begin to help.
I was hoping that your phone number would get things started but since that's not
recognized I hit a brick wall.
