A puk code is short for personal unlock key.
This is a master key for the sim card installed in the phone. If an incorrect pin number is XXXXX in three times in a row the phone will ask for the puk code. If the puk code is not entered in correctly in ten tries the phones sim card will be permanantly locked and you will have to purchase another one. If you no longer have the origonal puk code you willl need to contact your provider and they will give it to you.
