what causes a hot and ground reversal in an existing circuit?

It could be a few things or some of your receptacles could have gone faulty...........a loose connection or a broken wire on the neutral........ ............the speed connects are a pain in the neck sometimes not making good contact especially if a wire was removed from it at one time...........if wiring was done someone could have reversed the neutral somehow................if you have an outlet tester it would help you to pinpoint a questionable location
