I would agree that a dermatologist would be my next step. They will likely do a few things. First they'll do a skin scrape to rule out mites. They'll also do cytology to look for bacteria and yeast on the surface of the skin. They then do more invasive diagnostics like allergy testing and a skin biopsy. This is to look for allergies and auto-immune skin diseases.
If it's allergies, then immunotherapy is the next step to get it under control. Here is a nice write up on how that works:
