Customer: i have a 6yr old labrador, 49 kg, who had three episodes of E.canis/Babesiosis which were treated partially due to wrong diagnosis he has now a large spleen with sever anemia, which has recovered to a large extent with prednisolne, iron, B12 and diet. can Prednisolone help if its a large spleen due to partially treated babesiosis

Splenomegaly (enlarged spleen) is commonly seen with rickettsial diseases such as Ehrlichiosis as well as the hemoprotozoan parasite Babesia. Both infectious agents stimulate the immne system and the spleen is rich in immune-related cells. Hemangiosarcoma can arise in the spleen of a dog infected with these agents but can arise in any dog's spleen - usually at a median age of 8-10 years. Ultrasound is useful in clarifying why a spleen is enlarged. Ehrlichia and Babesia will cause a diffuse homogeneous change in the spleen while hemangiosarcoma usually forms discrete tumors.

Prednisone is the mainstay of therapy for an immune-mediated anemia particularly autoimmune hemolytic anemia (AIHA). It might be prescribed in refractory cases of rickettsial diseases or Babesia when an immune-mediated destruction of red blood cells is suspected. Prolonged immunosuppressive therapy with prednisone before specific antibabesial treatment can worsen outcome, however, and shouldn't be used in sick, hospitalized dogs.
