It sounds like the puppies are coming now. You're most likely seeing a sac hanging out of the vulva. This should result in a puppy being born within 1-2 hours of pushing. If the puppy does not come out, then I would worry it is breached. In that case, we can sometimes re-position it and get it out. Other times we have to go straight to c-section to get that puppy and the rest of the puppies out safely. It just depends on it's position, the fetal heart rate and how hard mom is contracting.
I would give it an hour to see if it progresses. If not, get to a vet for further care.
