My 13yr. old male dog just started to get up to move to another

My 13yr. old male dog just started to get up to move to another location in my room and fell over. He is trying to walk but keeps falling down. When I try to help him he looks at me like he doesn't know where he is or who I am. When he got up from the kitchen floor he had urinated there while lying down.

Bear is ataxic ("drunken sailor") and is exhibiting a change in mentation (mental status) which suggests encephalopathy (brain disorder). Stroke, brain tumor, and central vestibular (balance) disorder need to be considered at his age. Please give me an update on Bear.
