While I was clipping his hair, I noticed on our 10 yo Shih

While I was clipping his hair, I noticed on our 10 yo Shih Tzu that he has 3 black spots. 1 appears to be ulcerated and bleeds when touched. He has always had sensitive skin, we used Frontline on him but apparently too close to bathtime to be totally effective as he and the other 2 dogs have fleas again.

Is this likely something serious or is he probably okay? I am very poor and can't afford to take him to a vet unless it is something serious.

So sorry to hear he has these spots. May I ask where they are on his body?

Customer:

They are on his back. Two near his tail and one more toward his shoulder.

Dr. Altman :

Are they raised or within the skin?

Customer:

2 are flat, and just appear to be darkened skin. One is ulcerated, about the size of a pimple but when I try to manipulate it, it bleeds.

Dr. Altman :

How long ago did you first notice them and have they been about the same, better or worse?

Customer:

I only noticed them 2 days ago. There has been no change in that time. I did scrape off the loose blackened skin when I found them. He has long hair that I was clipping when I found these black spots.

Dr. Altman :

On a scale of one to ten, ten being the worst you can imagine in itching where would you put his discomfort?

Customer:

He is also very itchy. He is usually itchy but much more so now. 7.5

Dr. Altman :

What types of shampoos do you have at home? Do you have a medicated shampoo (hexadene) or an oatmeal-based shampoo?

Customer:

We have used a flea shampoo last but I made sure we have an oatmeal shampoo.

Customer:

He currently has fleas which we are working to get rid of.

Dr. Altman :

Great! Go ahead and bathe just those areas twice daily for a few days with ten minute contact time before rinsing then wipe off (full body with the itching every 2-3 days)
Also give benadryl at 1 mg/ pound every 8 hours for the itching as well
If you have a children's t-shirt you can put on him to prevent him from injuring the areas while they heal that would be great as well

If they are not improving over the weekend unfortunately I would advise a veterinary exam to ensure nothing more serious is going on that requires more direct treatment
