One of the big causes of this is a rectal mass. I would have your vet do a good rectal exam to see if there is a mass causing the narrowing of the colon/ rectum and pencil thin stool.

If he's intact, then an enlarged prostate may lead to this. Neutering is often curative.

Enlarged anal glands can also potentially lead to this- something else to have checked on rectal exam.
