Hello and welcome to JustAnswer!

You have a couple of different issues going on there. The gear ratio error codes along with lack of reverse do unfortunately point to a broken clutch hub or geartrain failure. The 2/4 and reverse clutches transmit power through the same hub to the geartrain.

When you take off from a stop the first time after starting the engine what happens when it should shift into second?
