The first thing that needs to be done is to see if the lift pump is supplying 500ml of fuel in 10 seconds to the injection pump. If good then you likely have a leaking injector or transfer tube.. To determine which one you would need to start capping off each cylinder until it start up easy then replace that injector. If you have never replaced the injectors now maybe the time. If you find 2 or more leaking I would replace all of them, and make sure and use Bosch and not the cheap-o injectors..

If you don't have any of the special tools to test it might be time to visit a diesel shop or dealership..

That is your problem. You need 6k of psi.. You will need to do a injector return test to see which one is leaking..

Not likely... First perform a injector return test, if excessive block off each injector until rail psi rises then replace that injector. Also the transfer tubes need to be torqued at 37 ft/lbs which will also cause high injector return.
