I have had a fungal infection on my right palm and fingers

I have had a fungal infection on my right palm and fingers for nearly a year. I have seen several G.P's and been prescribed creams and tablets. I am currently on a course of Itraconazole 100mg. So far there is no improvement and it is now appearing on my left Palm. I am allergic to terbinafine ointment. The appearance of palms is tiny bumps under the skin which become itchy, sore and red and the skin feels tight and flaking.
