You can be awake and have your teeth surgically removed (or cut out). As long as you have local anesthesia (the shots) then you will not feel any pain at all, and the process should be comfortable. Local anesthetic will numb you enough to where you do not need to be put to sleep. Afterwards, you will need to be on pain medication, however, and probably antibiotics as well to prevent infection.
