It was written in the court clerks office on the courts forms and then given to the judge by the court clertk for him either to grant or not grant a temporary protective order
If it is deliberate false information, then there could be perjury charges
Because such a form is taken out and sworn under Oath
However, it is rare that the DA will go for perjury charges in these matters unless it is something that happens time and time again
how would you file perjury charges.
You would need to talk to the DA about it.
But as I said it is rare that they will prosecute there matters, unfortunately
