I am submitting a Trial de Novo for a red light ticket. I submitted a TBD (Identity) but I was still found guilty. For item 3. ("I request a new trial for the following violations.." what should my wording be?

You would put the ticket number.

If the case is identity and it's not me but i don't want to identify the person, what's the best way to handle it?

The state has to prove that it is you and they cannot force you to testify. If it is not you then they cannot prove their case.
