You would typically retain an attorney in your area, licensed in that state to file a malpractice suit against the attorney. Generally like any other suit you just need to locate an attorney that handled attorney malpractice suits. There are usually less that specialize in that area of law so you might have to retain an attorney that is farther away say in another nearby town. You can generally contact the state bar. You can go to the Utah state bar here http://www.utahbar.org/directories/find-a-utah-lawyer/ put in your zip code and search for attorneys that practice in the area of malpractice. It is listed on the drop down box. I also find AVVO.com helpful to locate attorneys and at time read previous client reviews, etc.
As with most attorneys they will likely provide a 30 minute consultation. At that meeting you would want to bring all paperwork you have in case the attorney needs to review something and try to provide a fairly concise statement as to the facts that lead you to believe your attorney committed malpractice. If you can keep is fairly concise that will maximize the information you get back from the attorney during any free consultation.

I wanted to check in with you and confirm that you did not have any further follow up questions for me from the answers I provided to you on the 3rd.
