My doctor does not accept my insurance but offered to file it for me 'as a courtesy' - now I cannot get them to give me the money the insurance paid THEM! I paid them cash for each visit....

Yes. They filed the Ins for me and requested payment be made to their office.
You have a couple of choices. The first is to file a complaint with the state licensing agency. They help resolve fee disputes and in your case it is likely they will quickly give you the money back.The second way is to file a lawsuit, likely in small claims court. Again, I suspect that as soon as they get served with the papers they will pay you since they don't have any defense.
