The monitor in the cab showed an error code for #5 injector failure. I had just bought the machine used and when we pulled the cover we saw a number of remanufactured injectors had been installed.
My cat dealer said they are known to fail so we replaced them all.
The engine didn't blow exhaust before we replaced the injectors and on start up it was fine at first. Then a knocking noise and a lot of smoke.

If there are no codes, I would one of four things are causing your problem.
The first is an air system problem. This can be a turbo problem like fin damage, air filter restriction, or a leak in the air piping.
Second is a fuel pressure problem. It could be sucking air or have low pressure. The pressure should remain between 65-85 psi at all times.
Third is there is cylinder damage. Could be a broken ring or valve not sealing.
And Lastly is a faulty new injector.
If the air system is ok. I would have a cylinder cutout test performed to try and isolate the problem cylinder. Basically cut them off one at a time to see if the smoke stops. If you can isolate one, then you can swap injectors with another cylinder to see if the problem goes with the injector. If it does, the injector is bad. If not the cylinder has a problem.
