I can create PHP. I will need a .sql file, or you will need to tell me the database name, table name, and table layout, all the column headings. now I will create the PHP for $99 additional without handling file uploads, I see you have a file upload button $99 to add to database no file upload if you want file uploads it will be double that

the php file that you give can be linked to a website, and the mysql file will be in the server. And the information will be automatically filled

depends on how you have your mqsl setup. but yes, esentially, mysql is usally installed on a server along with php. And the PHP connects to it.
