I would like to create a web based form that the data gets collected in a mysql table, I have the html file and mysql file. I do not know how to make it a php to connect to mysql. The files are enclosed in these links please let me know how the file connects to mysql. Thankyou

I can create PHP. I will need a .sql file, or you will need to tell me the database name, table name, and table layout, all the column headings. now I will create the PHP for $99 additional without handling file uploads, I see you have a file upload button $99 to add to database no file upload if you want file uploads it will be double that

depends on how you have your mqsl setup. but yes, esentially, mysql is usally installed on a server along with php. And the PHP connects to it.
