I think i know WHY its giving you error message to insert disk.
Your USB UNIVERSAL media card reader does NOT support the HC (high capacity) media cards. And will ONLY recognize SD media cards up to 4GB in size, as this is the highest in size it goes before being considered a SDHC
Atleast this is my initial HUNCH, and most likely the answer, UNLESS...
This SDHC has worked in THIS universal reader in the past, and NOW it wont. Then that would be different. And woudl lead more to the a hardware failure instead. Either the reader or the SD card itself.
Here is all the types of cards it supports, I pulled this from the MANUAL. Unfortunately SDHC is NOT shown, therefore, it is NOT supported.
