While the youngest cats diagnosed with hyperthyroidism were 4 years of age, if you haven't seen a response to methimazole in 3 months, hyperthyroidism isn't the likely cause of your cat's polyphagia (increased hunger). The differential diagnoses for such a patient are diabetes mellitus (easily ruled out with a blood sugar test), Cushing's disease (very rare), and malaborption due to either severe inflammatory bowel disease (IBD) or small cell lymphoma in the gastrointestinal tract. My next diagnostic of choice for your cat would be an abdominal ultrasound looking for the inflammation associated with either IBD or lymphoma.

Do you recall what your cat's T4 and fT4 were prior to initiating methimazole therapy?
