Customer Question

I have chest pain, sharp pain, pressure, tightening, and difficulty breathing, its been happening frequently an im scared. I have like 20 other symptoms and abnormal blood work an doctors r thinking cancer or autoimmune disease but lupus panel negative. They suggested an angiogram catheter to measure in my heart. What r the chances that something that didnt show up on a chest xray, ct scan, ekg, or echo, would show up on an angiogram?

It might be a good chanceangiograms are the gold standard for looking at coronary vessels.
Customer: replied 8 months ago.
How often does it happen where nothing showes up on all other tests an they find something on the angiogram? What r the things that can b found during an angiogram? When I paid for this site it said it was already for unlimited questions?
Expert:  Dr. Phil, MD replied 8 months ago.
It would be unusualit doesn't happen oftenYou could find an aneurysm, a thormbosis, a tear or a stricture in a coronary artery
Customer: replied 8 months ago.
If my heart wasnt the actual problem what else could b the cause of these chest pains. An is an angiogram dangerous
Expert:  Dr. Phil, MD replied 8 months ago.
No not really dangerous in a young personrare risk of complicationsIt could be muscle, cartilage, stomach, etc
Customer: replied 8 months ago.
How do u determine which one it is once heart is ruled out
Expert:  Dr. Phil, MD replied 8 months ago.
You try medicine usuallystomach meds and see if it helps and so on
Customer: replied 8 months ago.
So meds first, then diagnosis? What other tests would b helpful to a cardiologist to asses my heart that I havent done
Expert:  Dr. Phil, MD replied 8 months ago.
That is pretty much it. There is not much else you can do heart wise in terms of testing
Customer: replied 8 months ago.
muscle and cartilage would be a chest wall problem, right? if it was a chest wall problem, what could be done to stop the sharp pains? would a chest wall problem cause difficulty breathing? if i have heart palpatations then wouldn't that indicate its a heart problem an not a chest wall problem?
Expert:  Dr. Phil, MD replied 8 months ago.
1)correct2)anti inflammatories usually work3)Yes, it could cause shortness of breath4)No it wouldn't cause palpitations
Customer: replied 8 months ago.
if heart problems don't cause heart palpitations what does? does chest wall problems cause heart to speed up and slow down? my triglicerides went from 104 to 327 in a month with no diet change, can that be something to worry about, especially with the other symptoms?
Expert:  Dr. Phil, MD replied 8 months ago.
1)many things. Palpitations can be totally benign too like PVCs. But dehydration, caffeine, thyroid can all cause palpitations. Anxiety too. 2)chest wall doesn't cause speed to change3)the triglycerides need addressed with diet/exercise but it doesn't relate to anything going on here.
Customer: replied 8 months ago.
why would my triglicerides go up so much so fast when my diet has been the same my whole life? also i was told that high triglicerides was an indicator of heart attack risk, is that true?
Expert:  Dr. Phil, MD replied 8 months ago.
1)I'm not sure why they went up so fast. 2)yes, they are an indicator but we are talking long term risk not short term
Customer: replied 8 months ago.
a positive centromere indicates crest syndrome, but doctors seem surprised that it goes negative sometimes. is it common for someone to have positive anti centromere and no have crest syndrome? and is it common for anticentromere to go back and forth between positive and negative?
Expert:  Dr. Phil, MD replied 8 months ago.
Yes, these tests are non specific
Expert:  Dr. Phil, MD replied 7 months ago.
I have opened the rating system for youPlease remember I only get credit if you rate positive for meIf you have more questions, just reply :)
Customer: replied 7 months ago.
What would b a reason for an angiogram to find something that ct, echi, ultrasound, an xray didnt find? During an angiogram, how long r they looking at ur heart? Isnt it true that in some cases, if ur heart has something irregular that itvmay not be seen at all times?
Expert:  Dr. Phil, MD replied 7 months ago.
Would you mind accepting additional services?
Expert:  Nadim Al-Mubarak replied 7 months ago.
Helllo .. I am Dr Mubarak, Board-certified US cardiologist with over 20 years experience Based on the information you provided, it appears that your doctor is suspecting a blockage in your coronary artery as possible source of your chest pain and so, he is recommending angiogram. The echo, CT, EKG and xrays do not allow him to see or evaluate the coronary arteries, the angiogram does that reliably. There are so many sources of chest pains, and the tests your listed exclude most of them but not a coronary blockage. The angiogram takes about 15 minutes with a quick recovery. It is a safe procedure. I hope this answers your question satisfactory, and if does does please complete my star rating. You may still ask me question about the same subject
Customer: replied 7 months ago.
is a coronary blockage the only thing that an angiogram shows that the other tests might miss? is an angiogram done while ur awake an will u feel anything? is there cases where the heart looks fine sometimes an other times it shows irregularities? isn't it possible that if my heart wasn't beating irregular or hurting, during the tests that it wouldn't show unless i was currently feeling something irregular? would it b more likely for the tests to show something if it was done while i was having an episode?
Expert:  Nadim Al-Mubarak replied 7 months ago.
Basically, yes, coronary blockages are the main interest, specially in your case. There are other rare findings to be looked for, but that would not exactly apply to you. The angiogram is typically painless, done under local with some sedation. You are very unlikely to feel any thing or remember any thing after it's completed. Irregularities in the heart beats are tested differently and are not tested by an angiogram. So, that is unlikely. Irregularities in the heart beats do not usually cause chest pain, but cause "flattering in your chest" or palpitations. But, that is not what the suspicion appears to be. A 30-day monitor is the test used to check the heart rhythm during these episodes. You may ask your doctor about it, I do not know what his/her thought process is.
Customer: replied 7 months ago.
so coronary blockages can't be seen on anything other than angiogram? is this often then answer when someone complains of heart problems and nothing is found on imaging? or is this extremely rare an almost not worth trying to look for coronary blockages? do coronary blockages happen in young people, an do they cause heart attacks? if a blockage is found what r the next options usually to fix the problem?
Expert:  Nadim Al-Mubarak replied 7 months ago.
I have to add though, that it is very rate that a 20 years old female would go for an coronary angiogram. You may also ask for coronary CTA which is CT angiogram of the coronary arteries, which is less-invasive than coronary angiogram and in your case would be more suitable, as your risk of having blockages is very low given your young age and being a female. In my almost 25 years old practice, I have not done an angiogram on some one your age, neither any of my colleagues
Expert:  Nadim Al-Mubarak replied 7 months ago.
Blockages, when found, most of the time can be fixed during the same procedure. Blockages are extremely rare in young females your age, yes. Your doctor could also be looking for congenital abnormalities of the coronary arteries, but you have not given me any information that would make him/her suspect that
Customer: replied 7 months ago.
what makes a ct angiogram different from a regular ct scan of your chest? what would make someone suspect congenital abnormalities
Expert:  Nadim Al-Mubarak replied 7 months ago.
CTA is a more suitable and less invasive test to consider than an angio. The angiogram allows direct visualization of the coronary arteries, and so blockages. Regular CT scan does not show the coronary arteries. Coronary CTA also allows good visualization of the coronary arteries
Customer: replied 7 months ago.
if i was given a 30 dAy monitor an recorded each episode what could be the possible out comes? is it possible for them to not see anything abnormal if I'm feeling a faster or slower heart beat? if they do see irregular rhythm, what is done next?
Expert:  Nadim Al-Mubarak replied 7 months ago.
There are many many possibilities and treatment depends on what they see. And, it is possible that they will not find any irregularities which is the most likely finding.
Expert:  Nadim Al-Mubarak replied 7 months ago.
I hope I answered your questions satisfactorily. I would appreciate it if you complete the star rating. Thats how I get compensated for my time
Customer: replied 7 months ago.
if my heart really is beating faster and slower at times to the point where someone who puts their hand on my chest can feel it, how could they not see it?
Expert:  Nadim Al-Mubarak replied 7 months ago.
It might still be normal and that's what they see on the monitor. And, the heart rate could still be within the normal limits
Customer: replied 7 months ago.
so it could show that it was a little fast but it could be with normal range. so if it was within normal range but shows a change can anything be done to fix it? is there a difference between speed of heart and rhythm of heart?
Customer: replied 7 months ago.
can u refer me to someone else please
Customer: replied 7 months ago.
hello u are currently taking up two specialist spots please refer me to someone else
Expert:  Dr. Phil, MD replied 7 months ago.
I'm here. Please, lets continue the conversation. The variation is speed is not concerning unless it is dramatic
Customer: replied 7 months ago.
so if its not dramatic, can they still do something to regulate it so it doesn't speed up and slow down
Expert:  Dr. Phil, MD replied 7 months ago.
yes
Customer: replied 7 months ago.
they said they found infiltrates in my chest X-ray, what could that mean
Customer: replied 7 months ago.
does a chest X-ray show both the heart and the lungs? can infiltrates be the white blood cells that r accumulating, infiltrated my chest? could chest infiltrates cause chest pain, trouble breathing, or heart problems? is everyones heart rhythm an speed the same pretty much, or people vary? can arthritis on an X-ray be mistaken for anything else or is it clear, like how a cyst, tumor, mass all can look the same? if i have all these crazy symptoms, wouldn't plaquenil, which is a drug used for lupus an arthritis, be only helpful to my weakness, fatigue, an muscle/ joint pain?
Expert:  Dr. Phil, MD replied 7 months ago.
Please don't forget a positive rating.Or accept additional services to continuethanks

i have several questions, please be sure to answer each, u can number them to b clear. 1)does a chest X-ray show both the heart and the lungs? 2) if X-ray only shows bones, then it would really show masses or anything right? 3) what can b found on a chest X-ray? 4) can infiltrates be the white blood cells that r accumulating, infiltrated my chest? 5) could chest infiltrates cause chest pain, trouble breathing, or heart problems?6) is everyones heart rhythm an speed the same pretty much, or people vary? 7) can arthritis on an X-ray be mistaken for anything else or is it clear, like how a cyst, tumor, mass all can look the same? 8) if i have all these crazy symptoms, wouldn't plaquenil, which is a drug used for lupus an arthritis, be only helpful to my weakness, fatigue, an muscle/ joint pain?

Hi,I'm Dr Chaithanya attending you now.Let me answer your questions.1. Yes, chest x-ray shows both heart and lungs2. Chest x-ray can be taken in two views, A-P view, and P-A view. Ap view can show bones and PA view can show soft tissues like lungs and heart3. Any abnormalities in lung, heart, spine, ribs can be found. 4. No it can't happen, white/red cells can't accumulate as infiltrates5. Yes, they can cause.6. It varies from person to person7. Arthritis cannot be detect by chest x-ray alone, it is a supportive investigation, blood tests and local examination is mandatory. Osteophytis and few features of arthritis are mostly specific, it is not possible that it is confused with other condition.8. Plaquenil is used in lupus arthritis, other drugs like NSAIDs, corticosteroids and Immune modulators can be used. The effectiveness of each drug on each patient is different and it should be adjusted based on individual susceptilbility for its efficiency and side effects. Hope this helps. Please feel free to ask if any further questions.
