Is it possible to put a 1997 2.5 in a 1999 plymouth breeze (originally a 2.4) without much, if any modifications?

The V6 from the donor will fit but there is a huge amount of work to do. you need the motor, transmission, complete wiring harness with all sensors and computers, exhaust system front to back and I believe front struts to carry the extra weight.

Is it simple??? the short answer is no. You could install a rebuilt motor for less money.
