This is a common problem on your transmission. It is usually caused by a p1811 code that is stored in the trans control module. It is caused by the computer seeing a longer shift than is normal. Once the long shift happens the computer will cause the pressure control solenoid to give max line fluid pressure, this is why it is shifting hard, the computer thinks the trans is going out so it will do this. And it usually only happens when the fluid in the trans reaches operating temp. When the fluid is cold it is thicker.
A trans rebuild is the only way to fix this problem. I had a similar vehicle in the past and drove it this way for years. So it does not necessarily need to be fixed right away.
