What is the problem when the vehicle will not shift out of drive? The lever simply goes up and down without changing the gear, as if it's come loose from the column.

Generally the shift cable end wears and pops off at the transmission lever. The end is not available separately and the shift cable itself must be replaced to do it properly. Temporarily you may be able to pop it back on and possibly secure it with a tie strap.
If you open the hood and look behind the air filter and down to the transmission (it's a bit buried under the air cleaner duct) you should be able to see the shift cable and where it attatches to the trans.
