Proof of Claim is a form that is filed by creditors in a Bankruptcy case. What is it that you would like to know about the Proof of Claim form?

Is this for a Chapter 7, 11, or 13 Bankruptcy?

This is for a chapter7

If, and only if, there are assets from the debtor that will be distributed to creditors, the Chapter 7 creditors will receive a notice in the mail to file a Proof of Claims form. The notice will also include the date by which the Proof of Claim must be filed. This date is usually 60 - 90 days after the notice is sent.

If there are no assets to distribute, the creditors are not supposed to file a Proof of Claim.
