It's possible that the DPF is restricted to a point that it cannot correctly regenerate, there may be a problem with a sensor preventing it from regeneration, or there may be an issue such as a failed injector which has caused excesive soot (which would also explain it running poorly).

The first step in diagnosing the problem will be to use a scan tool to see what codes have set. Do you know what codes are setting?
