A noise that only happens in certain gears is usually a gear or bearing issue in the transmission but if it only isn't happening in fourth gear then it may not be a trans issue. If it started happening in individually gears one at a time though then it may be an internal failure.

It's possible that the noise is actually related to powertrain movement rather than the transmission. Engine torque causes the engine and transmission to move in the mounts when more torque is applied and this can cause components to touch. If you have a broken or weak mount for instance it can cause excessive shift and components to touch that shouldn't.
