Have you replaced the oil feed line every time and also all of the breather hoses? The oil feed line is clogged, you must replace it in order to keep a steady flow of oil to the turbo. It is clogged with carbon.

would a clogged feed line cause eccessive shaft ware on the turbo ?

which seems to be on all the turbo's i replace !

also on the second turbo , it was replaced when a had the whole engine rebuilt , all because the oil pump shatted and push fine metal thru out the whole engine !

also is there anything else that could cause this fault ?

YES

you must replace the oil feed line with every new turbo and adhere to regular oil change intervals. If you allow sludge to build up you are going to be replacing turbos and engines constantly
