Hi. Have you tried to scan it?

Customer: replied 1 year ago.
I have a picture of it. Let meknow how to send it
Expert:  Ericka Pirtle replied 1 year ago.

Is the picture what you attached?

If not you can scan it two ways.

Expert:  Ericka Pirtle replied 1 year ago.

If you have a scanner you can open up the scanner program and do it that way. Or if you have a cell phone I can direct you to an app called cam scanner. You can take a picture of it using that app.
