Your bottle dates to late 'teens or early 1920s and was produced in Louisville, KY, by a contract bottler for Coca Cola called the Root Glass Company (incidentally famous for having produced the first “Hobbleskirt” shaped Coke bottles, circa 1915 or 1916).
Yours is quite a scarce bottle, but does turn up for sale from time to time at auction or on line (eBay) where it sells in the range of $20 - $40.
It therefore has a full retail value (if you saw it for sale in an antique store, say) of about $80. This is also the replacement value for insurance purposes.
