You have a silver plated (not sterling) creamer made by Derby Silver of Derby, Connecticut in pattern number 1121 that dates to circa 1880.

Quadruple plate means that the creamer is made of nickel, tin and zinc and is dipped four times in a thin wash of silver to give it the look of sterling .
