As for what that means... it could be two things, I think.
One: it's just an odd-looking entry, mostly.
Two: it's a symptom of a take-over or infection of your phone. I hope not.
What would be useful would be, for you to find a recent, or the most recent, entry that looks vaguely like that, and match the time and date on it (in the example you posted, that's Nov. 7 12:36) to what you recall doing at that time... preferably a time earlier today!
That would be the best way of identifying whether it is a text record, an email record, or a phone/voice call record. Or something spurious and viral/infection/etc.

This issue may be due to an app on the phone that is sending out data. If you wish, you may want to factory reset the phone. Doing so will erase everything on the phone including the apps and the content on the phone.
